Caring for you and your family has truly been one of the greatest privileges of our practice. After thoughtful consideration, Eyes on Queen Creek will be closing on October 17, 2026. This decision was not made lightly, and our team is committed to supporting you through a smooth and respectful transition.  Your health and continuity of care remain extremely important to us. Please contact us at (623) 274-2720 to obtain copies of your prescription and records and to request access to the patient portal.  Also, if you would like help identifying trusted local optometrists or ophthalmologists, our team will gladly provide recommendations based on your location, insurance, and clinical needs. Dr. Nicole Thomas of Gaze Optometry will be providing optometric services at the Queen Creek office beginning October 18, 2026.
